We’re looking for compassionate Hospice Registered Nurses who are dedicated to providing thoughtful, respectful care to patients and their families during the end‑of‑life journey. In this role, you’ll facilitate the development of individualized hospice plans of care in coordination with physicians and interdisciplinary team members. You’ll apply the nursing process of assessment, planning, implementation, and evaluation to the terminal disease process while honoring the medical, psychological, cultural, social, and spiritual needs of each patient.
You’ll be accountable for assigned patient care, accurate clinical documentation within the electronic medical record, and active participation in interdisciplinary team activities to enhance care delivery in the hospice setting.
Patient Care & Treatment- Facilitate development and implementation of hospice plans of care in collaboration with physicians and interdisciplinary team members
- Provide nursing care aligned with hospice philosophy and the terminal disease process
- Incorporate medical, psychological, cultural, social, and spiritual needs into patient care
- Apply assessment, planning, implementation, and evaluation throughout the continuum of hospice care
- Deliver care with professionalism, empathy, and respect for patients and caregivers
This role involves working closely with an interdisciplinary hospice team to improve care delivery and ensure a supportive, patient-centered approach. The Hospice RN contributes to coordinated care efforts while maintaining professional boundaries and fostering trust with patients, caregivers, and team members.
Qualifications- Graduate of an accredited school of nursing
- Current California RN license
- Minimum one (1) year of clinical practice experience
- Current BLS certification (maintained thereafter)
- Satisfactory completion of a pharmacology test with a minimum score of 80% (if new hire to RRH)
- Valid California driver’s license, proof of good driving record, and automobile insurance
- Access to a dependable vehicle for travel to patient locations
- Ability to read, write, and comprehend the English language
- Ability to work cooperatively with an interdisciplinary team
- Ability to relate professionally to patients and caregivers, including maintaining professional boundaries
- Ability to accurately locate, review, and document clinical notes in an electronic medical record
- Ability to accept and apply hospice philosophy to clinical practice
