Description
The Enhanced Care Manager – Licensed Vocational Nurse (LVN) facilitates patient continuity of care within the ECM healthcare team. Under the clinical direction of the Primary Care Physician and Registered Nurse, the LVN coordinates care for patients needing support, collaborating with patients, families, physicians, nurses, and other ECM team members to identify needs and expedite appropriate, cost‑effective care.
Key Responsibilities- Implement and support individualized patient care plans.
- Address patient risk factors and obstacles to care, including hospital and ER utilization.
- Facilitate engagement between the PCP and patient.
- Demonstrate ability to influence others while motivating them to change.
- Participate in required care coordination conferences.
- Maintain a predetermined patient caseload.
- Exercise sound clinical judgment; seek advice and sign off with a Registered Nurse as appropriate.
- Take initiative to find solutions to difficult and sensitive problems.
- Participate in quality gap closures.
- Act as a patient advocate.
- Perform miscellaneous job‑related duties as assigned.
- Willingness and ability to interact with healthcare personnel in diverse clinical settings.
- Understanding of HIPAA, confidentiality obligations, and nursing best practices.
- Teamwork and interpersonal skills.
- Excellent organizational, time‑management, and multitasking abilities.
- Ability to review clinical data to address care opportunities and utilization trends.
- Critical thinking and adaptability to a fast, complex environment.
- Strong autonomous problem‑solving skills.
- Documentation accuracy in medical records.
- Adaptability to new technologies and software.
Regularly required to walk, bend, sit, stand, use hands and arms, and lift or move 10–25 pounds. Requires close vision and ability to adjust focus. Occasional travel to healthcare practices may be required. Normal office environment noise level is low to moderate.
Qualifications- Current Vocational Nurse License.
- Minimum of 2 years of experience in ambulatory care management or related settings.
- Valid driver’s license and ability to travel.

Kern County, CA. Travel: up to 75% of work may involve travel.
- Salary Range: $72,000 – $90,000 per year.
