RVNAhealth is seeking an experienced Hospice Nurse Liaison (RN) to serve as both a clinical expert and business development professional, helping patients gain timely access to compassionate hospice services throughout Fairfield County, Connecticut.
In this highly visible, community-based role, you'll partner with hospitals, skilled nursing facilities, assisted living communities, physician practices, oncology programs, and other healthcare providers to educate referral partners, identify hospice-appropriate patients, and facilitate seamless transitions into hospice care.

What You'll DoAs the Hospice Nurse Liaison, you will serve as a trusted clinical resource while developing and strengthening referral relationships throughout the community.
Relationship Development & Business Growth- Develop and maintain strong partnerships with hospitals, physicians, case managers, discharge planners, skilled nursing facilities, assisted living communities, oncology practices, and other referral sources.
- Conduct regular outreach and facility visits to strengthen relationships and identify opportunities for hospice referrals.
- Educate healthcare professionals on hospice philosophy, eligibility guidelines, Medicare hospice benefits, symptom management, and RVNAhealth services.
- Represent RVNAhealth at community events, networking functions, healthcare meetings, and professional organizations.
- Identify new referral opportunities and support strategic hospice growth initiatives.
- Collaborate with leadership to achieve referral, census, and market development goals.
- Identify patients who may benefit from hospice services based on disease progression and goals of care.
- Collaborate with physicians and interdisciplinary care teams to facilitate timely hospice referrals and admissions.
- Educate patients and families regarding hospice services, benefits, and end-of-life care options.
- Support smooth transitions from hospitals, skilled nursing facilities, assisted living communities, and physician practices into hospice care.
- Serve as a clinical resource regarding hospice eligibility, symptom management, advance care planning, and Medicare hospice benefits.
- Coordinate closely with intake, admissions, and hospice clinical teams to ensure an exceptional referral-to-admission experience.
- Monitor referral trends and identify opportunities to increase hospice awareness and utilization.
- Maintain accurate referral documentation and activity within CRM and agency systems.
- Provide leadership with market intelligence, referral feedback, and community insights.
- Support educational initiatives focused on hospice, serious illness management, caregiver support, and advance care planning.
Required Qualifications
- Current Connecticut Registered Nurse (RN) license.
- Minimum of three years of clinical nursing experience.
- Valid driver's license and reliable transportation.
- Ability to travel throughout RVNAhealth's service area.
- Experience in hospice, palliative care, home health, oncology, geriatrics, care transitions, or case management.
- Previous healthcare business development, physician relations, hospice marketing, or community outreach experience.
- Bachelor's degree in Nursing, Healthcare Administration, Business, or a related field.
- Working knowledge of hospice eligibility criteria and the Medicare Hospice Benefit.
- Outstanding relationship-building and networking abilities.
- Strong consultative sales and business development skills.
- Excellent communication and presentation skills.
- Compassion and confidence discussing serious illness and end-of-life care.
- Strong organizational, analytical, and problem-solving skills.
- Ability to work independently while managing multiple referral relationships.
- Proficiency with Microsoft Office and CRM or referral management systems.
