As a Mobile Health Clinician –NY at Doc Go, you will provide services in New York City by monitoring, assessing, and delivering directed treatment for common injuries and illnesses. You will drive to patients’ homes to provide care and monitoring, engage in assessments, and provide treatment under the supervision of an Advanced Practice Provider (APP) for patients facing barriers to accessing traditional healthcare.
You will maintain patient medical records and documentation consistent with state regulations and Doc Go standards via HealthPoint and Athena EMR.
- Act as a conduit for the medical treatment of the Health Plan Partnerships patient population
- Navigate conversations related to management of health, well‑being, medication management, and goals of care
- Provide supervised, directed treatment of common medical diagnoses
- Participate in continuing education as required by the state where you reside and the Doc Go training team
- Adhere to clinical and safety standards, protocols, and performance metrics
- Provide care with cultural sensitivity and compassion
- Lead your practice with a patient‑centered approach
- Other related duties as deemed necessary
- Current unrestricted NY State LPN license
- Phlebotomy experience
- Minimum of two years of experience as an LPN
- Active and current American Heart Association BLS certification
- Valid driver’s license in state of residency with 2+ years of driving experience and acceptable driving record
- Ability to lift 50–60 lbs
- Experience in Emergency Department, Urgent Care, Internal Medicine, Hospital – Critical Care, Family Practice, or other relevant areas preferred
- Clinical work experience with pediatrics preferred
- Fluency in English/Spanish preferred
