Director of Health Services (DHS)| RN / LPN
We are seeking an experienced Director of Health Services / Wellness Director to lead clinical care in our Assisted Living and Memory Care community. This leadership role is ideal for a nurse who has built their career in Senior Living (Assisted Living, Independent Living, or Memory Care) and brings a strong track record of team leadership, regulatory compliance, and quality resident outcomes.
We’re looking for a steady, relationship-driven clinical leader who values consistency, team development, and long‑term community impact — not short‑term assignments.
This role oversees all clinical and resident care services within an Assisted Living and Memory Care setting, including care planning, medication management, survey readiness, staff supervision, and collaboration with families and providers. Experience specifically within senior living (AL/MC/IL) environments is strongly preferred.
Responsibilities- Delegates tasks according to NM Board of Nursing Rules and Regulations (ensures a written physicians order, assess the resident condition as stable, ensure the caregiver is capable of performing the task by observation of that caregiver’s performance, ensures training of the task to the caregiver, provides written instructions and a return demonstration of the task, documents the task delegated on the resident’s service plan).
- Develop and maintain a good rapport with all parties involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of
- Report known or suspected incidents of fraud, waste, and or abuse to the Executive
- Review the 24‑hour report and health monitoring and make recommendations as needed
- Assess the use of supportive devices with restraining qualities in accordance to established standards of
- Plan, develop, organize, implement, evaluate, and direct the health service department, as well as its programs and activities, in accordance with current Oregon Administrative Regulations and OSHA guidelines
- Participate in the coordination of care with outside and offsite providers
- Participate in the Continuous Quality Improvement Assist in developing improvement plans to correct identified problem areas
- Participate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directed
- Coordinate services amongst the Interdisciplinary Team
- Ensure that all personnel involved in providing care to the resident are aware of the resident’s care Ensure that nursing personnel refer to the resident’s care plan prior to administering daily care to the resident
- Implement and follow company policies and guidelines
- Review IR reports and assist with the investigation and recommendations as needed
- Complete Alert Log and Alert Follow guidelines for Alert Charting criteria
- Review all physician orders (triple check system) timely
- Complete Change in condition assessments per company policy
- Complete Quarterly assessments per company policy
- Complete admission assessments required by Registered Nurse per company policy
- Follow guidelines for skin tracking, monitoring, and treatments
- Participate in all compliance training programs
- Participate in developing the Service Plan when residents receive health Make recommendations for service plans as needed
- Attend and participate in care conferences as invited by Executive Director
- Possess, as a minimum, a Nursing Degree from an accredited college or university
- Must have, as a minimum, three (3) years of experience in a supervisory / leadership role, preferably in Senior Living (AL/IL, MC or SNF)
- Must have, as a minimum six (6) months of experience in rehabilitative and restorative nursing practices
- Possess a current, unencumbered, active license to practice as a Registered Nurse or LPN in this state
