In-Home Health - Nurse Practitioner or Physician Assistant Greeley

• In‑home health evaluations lasting approximately 45 minutes, including medication and medical history review, physical assessment, and optional diagnostic tests such as diabetic eye exams or spirometry.
• Use of an iPad and an intuitive clinical workflow for each evaluation; no prescribing, lab ordering or treatment changes required.

• Engage with patients in the comfort of their own homes to build trusted relationships and help improve health outcomes.
• Contribute to nationwide efforts to visit millions of people, connecting them to the right care for their unique needs.
• Travel locally and, when licensed, across states as needed to provide in‑home services.
• Conduct virtual visits when necessary.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry and diabetic eye exams.
• Operate within diverse home environments, managing potential allergens, pets or secondhand smoke while maintaining patient safety.

• Active, unrestricted NP or PA license(s) in the coverage area(s).
• Board certification (required); multi‑state licenses (preferred).
• Ability to cover the primary locations of Greely and surrounding area.

The typical pay range for this role is: $87,035.00 – $. The actual base salary will depend on experience, education, geography and other factors.
