Location: Gallup, NM

Pay: $1,763 per week
Shift Information: 3 days x 12 hours
Contract Duration: 9 Weeks

About the Position

Under the direction of the RN Nursing Supervisor, Unit Manager, ADON or ADON – Unit, or Center Nurse Executive (Nurse Leader), the Registered Nurse (RN) delivers efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction. He/she operates within the scope of practice defined by the State Nurse Practice Act and delegates aspects of patient care to LPNs and CNAs consistent with their scope of practice.

The RN manages patient care by performing nursing assessments and collaborating with the nursing team and other disciplines, patients and families to develop effective plans of care.

Responsibilities
  • Assessment:
    Collects, validates, synthesizes, and records objective and subjective data; prioritizes data collection; analyzes all data; identifies actual or potential problems or needs based on data analysis.
  • Care Planning:
    Establishes realistic and measurable short and long-term goals; develops individualized interventions; determines timelines; conducts systematic ongoing evaluation of patient outcomes; reviews and revises plan as indicated.
  • Implementation and Care Coordination:
    Performs nursing functions within scope; coordinates and delegates care; verifies accuracy of medical orders; administers medications and treatments per physician orders; communicates and documents interventions and patient responses.
  • Managing Patient Care:
    Delegates care responsibilities considering complexity of care, competency of staff, center policies, and state practice act; supervises staff to ensure delegated tasks and interventions are implemented appropriately per policy; evaluates and adjusts plan of care to achieve goals and minimize rehospitalizations; actively participates in utilization or interdisciplinary care management meetings; communicates discharge recommendations, prepares patients/families for discharge, facilitates shift-to-shift communication, uses PCC, performs drug count at shift begin/end.
  • Collaboration and

    Education:

    Collaborates with physicians during rounds; coordinates with other departments to provide timely effective care; contributes to a respectful, team-oriented environment; attends mandated in-service and external professional education; assists with orientation of newly hired staff; promotes culture of safety; maintains confidentiality and protects PHI; works beyond scheduled shift if needed.

  • Compliance
  • Complies with applicable legal requirements, standards, policies and procedures, including the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act, and HIPAA.
  • Participates in required orientation and training programs.
  • Promptly reports concerns and suspected incidences of non‑compliance to supervisor, Compliance Liaison, or Compliance Officer via the Integrity Hotline.
  • Cooperates with monitoring and audit functions and investigations.
  • Participates, as requested, in quality assurance and process improvement activities.
  • Educational and Vocational Requirements
  • Graduates of an approved school of nursing.
  • Licensed by the State Board of Nursing.
  • Preferred experience in rehabilitative or geriatric nursing.
  • Proficiency in reading, writing, speaking, and understanding English; ensures safety and wellbeing of patients and visitors.
  • Verification of TST (tuberculin skin test) as required by state law; TST administered at the work site if required.
  • Maintains current BLS/CPR certification (All SNFs and Behavioral Centers; ALFs/ILFs as required by state).
  • Job Skills
  • Current physical assessment skills and comprehensive knowledge of nursing principles; ability to recognize and manage emergency situations.
  • Knowledge of medications, proper dosage, and expected results.
  • Ability to coordinate, delegate, and supervise nursing functions; problem solve and make decisions.
  • Ability to create a patient‑centered environment.
  • Ability to communicate in English orally and in writing.
  • Basic computer knowledge.
