  Skilled nursing facility (long-term care)

• Administer prescribed medications and treatments following physician orders and patient care plans
• Monitor resident health status, document changes, and report concerns promptly to the supervising RN
• Perform basic nursing procedures, wound care, and routine assessments as assigned
• Assist residents with activities of daily living while promoting safety, dignity, and comfort
• Maintain accurate clinical documentation in the electronic medical record; ensure timely communication with the care team
• Respond to call lights, triage needs, and coordinate with the multidisciplinary team to adjust care plans

• Active Licensed Practical Nurse license in New Mexico (in good standing)
• Current CPR certification (BLS) or as required by facility policy
• Ability to work a 3x12 night schedule and comply with shift rotations
• Experience in or willingness to work in a long‑term care/geriatric setting
• Completion of an approved practical nursing education program
• Competence in basic nursing procedures and resident safety in a skilled nursing care environment
