LVN ​/ LPN – LTC​/SNF

Job Summary:

Provide quality nursing care to residents in a long-term care or skilled nursing facility setting as an LVN/LPN. Ensure residents’ needs are met through compassionate and professional assistance.

Responsibilities:

• Administer medication and treatments according to doctor’s orders.
• Monitor and document residents’ health status and report any changes.
• Assist with daily living activities such as bathing, dressing, and feeding.
• Follow facility protocols and maintain accurate documentation.
• Support the healthcare team in implementing care plans.
• Ensure compliance with all safety and regulatory standards.

Qualifications:

• Minimum of LVN or LPN license in the state.
• Proven experience in LTC/SNF or similar environment.
• Ability to assess and respond to patient needs accurately.
• Strong communication and interpersonal skills.
• Knowledge of medication administration and documentation procedures.
• Ability to work effectively in a team-oriented setting.
