Responsibilities
  • Manage and coordinate caseloads, ensuring timely case progression and documentation.
  • Maintain accurate records in case management system and ensure data integrity.
  • Monitor and report on key performance indicators and productivity pipelines.
  • Ensure compliance with applicable policies, regulations, and ethical standards.
  • Develop individualized plans to advance client outcomes within defined milestones.
  • Collaborate with cross‑functional teams to align services with client goals.
  • Identify barriers to service access and coordinate resources to resolve them.
  • Provide guided support and coaching to clients to foster self‑sufficiency.
  • Prepare and deliver regular status updates to supervisors and stakeholders.
  • Proactively identify opportunities for process improvements and efficiency gains.
  • Maintain professional development through trainings to support growth into advanced roles.
Experience / Qualifications
  • Experience managing caseloads and coordinating case progression.
  • Proficiency with case management systems and data integrity practices.
  • Strong knowledge of applicable policies, regulations, and ethics in care.
  • Ability to monitor KPIs, productivity pipelines, and report trends.
  • Capability to develop individualized client plans with milestones.
  • Collaborative skills to align services with client goals across teams.
  • Identify barriers to access and coordinate resource referrals.
  • Result‑driven mindset with ability to meet targets in a high‑visibility environment.
  • Provide coaching to foster client self‑sufficiency and empowerment.
  • Prepare regular status updates for supervisors and stakeholders.
  • Identify and implement process improvements for efficiency gains.
  • Commitment to ongoing professional development and growth into advanced roles.
  • Ability to work on‑site with strict adherence to workplace policies.
