Hospice RN Case Manager - Where Support Meets Impact
- Lower patient-to-nurse ratios for meaningful, unrushed hospice care
- Strong interdisciplinary team support; collaborative care with Social Work, Chaplaincy, Aides, and providers, so you’re never practicing alone

Location:
1028 S. Main Street Suite C, Fond du Lac, WI 54935. Coverage area:
Serving Winnebago & Fond du Lac Counties; travel throughout the territory is required.
Schedule:
Monday‑Friday 8AM‑5PM.

- Assess/monitor physical, emotional, and psychological needs of patients

- Direct nursing care: administering medications, treatments, and interventions
- Provide pain and symptom management
- Educate and support the patient’s family and caregivers
- Collaborate with interdisciplinary team

- Participate in on‑call rotation as required by local branch

Qualifications- Graduate of an accredited nursing school with current licensure to practice in the state of operation
- One year nursing experience required; 3+ years preferred
- Experience in hospice or a similar setting preferred
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
