Salary Range: $50,000.00 To $52,000.00 Annually
The Case Manager plays a key role in engaging and supporting youth through program enrollment, individualized coaching, regular check-ins, activity planning, workshop facilitation, advocacy, and referrals to community resources. Programming focuses on academic exploration, career planning, life skills, professional development, and community engagement. Working closely with the Program Manager and other program staff, the Case Manager will help ensure youth have access to essential resources and services while creating opportunities to build meaningful connections, strengthen socioemotional skills, and develop the tools needed for lifelong learning and success.
This position requires a strong commitment to youth development and the ability to inspire and empower young people.
This full-time position is 40 hours per week, with four days on-site and one day of virtual work available after a successful 60-day performance review.

Key Responsibilities:
Youth Engagement & Case Management:
- Conduct assessments for new youth and develop an IDP. Update IDP at least every 90 days with service planning on short and long-term goals.
- Manage a caseload of youth participants, providing regular check-ins to identify barriers to success and create space to develop goals and career planning.
- Build meaningful relationships with youth using Positive Youth Development (PYD) principles to understand and support their strengths, interests, goals, and needs.
- Make appropriate internal and external referrals and follow up to ensure youth receive required services in a timely manner.
- Coordinate with service providers, case managers, and other partners to support continuity of services and ensure youth have access to available resources.
Data Management & Documentation:
- Prepare, organize, and maintain accurate case files for each participant. Regularly enter and update participant engagement and service data in the Efforts to Outcome (ETO) database.
