About the RoleWe're seeking a skilled Occupational Therapist (OT) to provide in-home therapy services as prescribed by the attending physician. This role includes evaluating patient needs, implementing treatment plans, and ensuring safe, high-quality care in the home environment.
What You'll Do- Provide occupational therapy treatments as ordered by the physician
- Assist in developing and updating the Plan of Care (POC)
- Evaluate patient functional levels and progress
- Observe, record, and report patient condition changes
- Teach and guide family members and Lorian staff on therapy programs
- Supervise other healthcare personnel when appropriate
- Assess the home environment and recommend modifications
- Participate in in-service education programs
- Complete and submit accurate, timely clinical documentation per Lorian policies
Note:
Documentation must be accurate and complete for visits to be payable.
- Graduate of an accredited occupational therapy program
- Registered with the Occupational Therapist Association; licensed in state of practice (as applicable)
- Minimum 2 years of relevant OT experience
- Pre-employment health clearance, including TB test/CXR (per state requirements)
- Able to read, follow written instructions, and document care accurately
- Self-directed with strong independent work skills
- Current CPR certification
- Professional, flexible, and demonstrates tact and patience
- Good physical stamina and mental health
- Valid driver's license and insured personal vehicle (per Lorian Health requirements)
- Able to document all client-related activity

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $90 to $120 visit commensurate with experience and depending on type of visit.
