I) to join our team of dedicated clinicians.

Job Responsibilities
  • Initiates, develops, and implements intermediate nursing care plans, evaluating patient progress toward goals.
  • Provides direct patient care, performs treatments, administers medications, and educates patients and families/caregivers.
  • Modifies care plans in response to changing patient status or physician orders to achieve established or revised care goals.
  • Assesses patient needs and physical status at each skillful visit through health data access and patient interview.
  • Obtains and clarifies physician orders for plan‑of‑care revisions; informs physicians promptly of significant changes in patient condition and provides written summaries.
  • Provides training and mentorship to Infusion Nurse

    I’s as needed to ensure patient safety and regulatory compliance.
  • Effectively communicates with the clinical team, providers, patients, and families to facilitate continuity of care.
  • Prepares real‑time clinical documentation during visits and submits to the nursing department in compliance with billing timelines.
  • Coordinates discharge planning and prepares discharge summaries, reporting patient care needs, progress, and goals upon transfer.
  • Demonstrates compliance with agency operations, policies, procedures, professional standards, and regulatory requirements.
  • Maximizes work efficiency through the use of computers and automation technologies to validate orders and document care.
  • Responds promptly and appropriately to patient requests and initiates emergency procedures as necessary.
  • Follows established programs and practice to reflect Option Care’s mission, values, and objectives.
  • Observes legal and ethical guidelines for safeguarding patient confidentiality and adheres to HIPAA regulations.
  • Instructs patients about scope of services and related financial obligations for care and service charges.
  • Attends required training modules, in‑services, and continuing education to maintain competency and professional licensure.
  • Provides oversight and coordination of paraprofessionals in the home setting and may act as Supervisor in the absence of the Supervisor Nurse Manager.
  • Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, onboarding, and supervisory activities.
  • Participates in nursing department on‑call responsibilities.
  • Performs other related duties as directed by supervisor.
Qualifications
  • Active and unrestricted RN license in the state of practice.
  • Minimum of 1 year of nursing experience.
  • Current CPR certification required.
  • Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs.
  • Advanced certification and training as applicable.
  • Required licensure to operate a motor vehicle in the state of practice with proof of liability insurance.
  • Basic knowledge of computer operating systems and software applications and ability to self‑evaluate for annual appraisal and set professional growth goals with nurse manager.
Physical Demands

Ability to lift up to 75 pounds, exert up to 20 pounds of force occasionally, and perform frequent lightweight lifting. Duties may include walking, standing, crouching, kneeling, turning, balancing, stooping, reaching overhead, and other movements typical of a home infusion setting. Candidates must meet occupational exposure standards for blood, body fluids, hazardous substances, and environmental hazards. Safety requirements include closed‑toe shoes, face mask, goggles, gown, gloves, and a functional communication device.

Requirements

Willingness to travel up to 100% of the time for business purposes.

Preferred Qualifications & Interests (PQIs)
  • Bachelor of Science in Nursing preferred.
  • 1–2 years of previous infusion nursing experience preferred.
