Inpatient Palliative Care Nurse Practitioner / Physician Assistant Palliative Care | California
We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management, pain control, and quality of life for individuals with serious or chronic conditions. This role supports inpatient palliative care, community-based care, and home visits, with flexibility to practice across settings based on patient needs and program demand.

- Provide comprehensive palliative care consultations across inpatient, community, and home-based settings
- Develop and manage inpatient consultation emphasizing comfort, symptom control, and patient goals
- Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams
- Serve as a clinical leader within the interdisciplinary team
- Educate patients and families on disease progression, treatment options, and goals of care
- Demonstrate strong time management while managing a diverse patient caseload
- Participate in interdisciplinary meetings, case conferences, and care planning
- Active California Nurse Practitioner or Physician Assistant license
- Strong interdisciplinary team leadership skills
- Excellent time management and organizational abilities
- Comfortable with home visits and community driving
- Strong communication skills with patients, families, and care teams
- Ability to work independently while coordinating care across settings
- Must have driver's license and reliable source of transportation and insurance
- Credentialed at either Orange County, LA County, Long Beach, Temecula Valley, Riverside, San Bernardino, San Diego County area hospitals

Hourly range for posted region: $85 - $120 USD
