Neighborhood Healthcare PACE is seeking a Utilization Management and Transitional Care Nurse Navigator/Liaison to strengthen relationships with hospitals, SNFs, and other care providers for smooth transitions of care. This on-site role supports PACE participants, with a Monday–Friday 8am–5pm schedule.
The position requires a CA RN license and at least 3 years of clinical nursing experience, including transitional care or case management. BLS and CPR/First Aid certifications are required.
