Riverwalk Post Acute is seeking a compassionate, organized, and motivated Social Services Assistant to join our dedicated interdisciplinary team. In this role, you will support residents and families by helping coordinate services, addressing psychosocial needs, and assisting with discharge planning and resident advocacy. This is a rewarding opportunity to make a meaningful impact in the lives of our patients and residents every day.
Key Responsibilities Assist the Social Services Director with resident assessments and documentation.
Support residents and families with emotional, social, and practical needs.
Help coordinate discharge planning and community resources.
Communicate effectively with residents, families, physicians, and interdisciplinary team members.
Assist with care conferences and resident/family meetings.
Maintain accurate and timely records in accordance with facility policies and regulatory requirements.
Support resident rights initiatives and advocate for resident well-being.
Assist with admissions, transfers, and discharge processes as needed.
Participate in facility events, activities, and quality improvement initiatives.
Qualifications High school diploma or equivalent required.
Previous experience in healthcare, skilled nursing, social services, case management, or customer service preferred.
Strong organizational, communication, and interpersonal skills.
Compassionate approach to resident care and family support.
Ability to multitask and work effectively in a fast-paced environment.
Proficient with computer systems and documentation.
