Job DescriptionBilingual Community Health Worker –Inland Empire & Desert Regions (Travel Role)
Heritage Health Network (HHN)
Join Heritage Health Network as a Bilingual Community Health Worker (CHW) and help connect Medi-Cal members to care through our Enhanced Care Management (ECM) program.
This hybrid role requires 3 days in the field per week at community locations (food banks, shelters, clinics, churches, probation offices, hospitals) and 2–3 days conducting telehealth outreach onsite in Riverside, CA
. Spanish fluency is required.
- Conduct telehealth outreach to members and screen for ECM eligibility
- Submit referrals to health plans and document interactions
- Engage in field outreach to identify and support potential ECM participants
- Build relationships with community partners and assist members in accessing healthcare and social resources
- High school diploma required; BA or Master’s in Public Health, Social Work, or related field preferred
- Experience in community outreach, social services, or healthcare navigation preferred
- Valid driver’s license, active auto insurance, and reliable vehicle for field work
- Bilingual in Spanish and English
- Strong communication, organization, and time management skills
