Heritage Health Network (HHN) seeks a bilingual Community Health Worker to connect Medi-Cal members to care through the ECM program. Based in Riverside, this travel role supports Inland Empire & Desert Regions with in-person field outreach and telehealth coordination.
You will work three days in the field at community locations and two to three days from telehealth sites, building partnerships with clinics, shelters, and social services.
