Heritage Health Network (HHN) is seeking a Part-Time Registered Nurse Care Manager (RNCM) to provide clinical oversight to a team within our Enhanced Care Management (ECM) program.
Location:
Hybrid / Field-Based; remote and in-person engagement as needed.
The RNCM will review member charts, assess care plans, guide non-clinical Lead Care Managers, participate in weekly case reviews, and ensure Medi-Cal ECM guidelines compliance. Flexible hours with one weekly case review meeting.
