• Provide skilled and compassionate care to patients in their homes, focusing on their physical, emotional, and social needs

• Assist patients with daily living activities, such as bathing, dressing, and grooming

• Monitor and document patient progress, reporting any changes or concerns to the healthcare team

• Develop and implement personalized care plans to meet the unique needs of each patient

• Collaborate with other healthcare professionals to ensure seamless care coordination

• Current certification as a Home Health Aide (HHA)

• High school diploma or equivalent required; education and experience in a related field (nursing, social work, or healthcare) a plus

• Ability to work flexible schedules, including weekends and holidays

• Strong communication and interpersonal skills
