Acute Care RN: Care Coordination & Patient Outcomes

Overview

The RN's purpose is to provide quality nursing care to our patients, coordinate care with ancillary staff, and communicate effectively with physicians. This position contributes to the department’s goals by being accountable for the delivery and coordination of patient care at a demonstrated level of safe practice and within the scope of practice as defined by the Florida State Board of Nursing and TGH policy.

Along with other members of a care team, this role is responsible for patient outcomes that meet the high standards of care provided by TGH.

• Act as a clinical and/or emotional resource for the clinical team and provide guidance for complex patients to ensure the highest quality of clinical outcomes.
• Perform thorough physical and psycho‑social assessments, identify potential barriers for social determination of health, manage patient medication profiles, and efficiently communicate with the provider/ lab & test results to ensure a smooth and positive patient experience.
• May complete triage/in‑basket messages, annual wellness visits, care management, and coordination of chronic conditions to support the care team and ensure overall continuity of care.
• Provide patient education, including reviewing a plan of care if protocols are in place, ensuring the patient has a thorough understanding of care.
• Maintain records of medical and nursing treatments and related services executed by nursing staff through appropriate and accurate documentation in the electronic medical record, resulting in compliant and useful documentation.
• Maintain overall patient experience and safety, following all emergency clinical policies and protocols to allow for optimal patient care.

• Degree in Nursing.
• Previous nursing experience in an acute setting preferred.
