Program Overview

As an OT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program. In a world of shorter SNF stays, TYTH is a unique program where you as the clinician work directly with patients in the SNF and home health, providing a continuity of care that allows for you to have a greater impact on patient outcomes.

You will be primarily in the home health setting with dedicated time and caseload support for the SNF, covering the surrounding area.

Rivetus has been serving the geriatric post‑acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians, providing continuity of care across Skilled Nursing, Home Health, and Outpatient settings.

• Successful completion of an accredited Occupational Therapy program.
• Licensure as required by the State of Michigan.

• Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
• Initiative and flexibility in the work environment.
• Ability to react effectively to the needs of patients and their families.
• Physical ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
