Provides safe and effective drug therapy and clinical patient care services to ensure optimal outcomes under the direct supervision of the respective preceptor. Ensures implementation of the pharmaceutical care plan by assuring the accurate and timely distribution of medications to patients. Serves as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as to patients.

• Reviews, evaluates, and processes medication orders for demographic data (e.g., gender, age, andweight), appropriateness of therapy, dose, frequency, route, dosage form,interactions, allergies, related laboratory values, and possible drug interactions.
• Provides direct patient careby actively participating in rounds, reviewing patient charts, and developing and implementing patient - specific pharmacotherapy plan to ensure optimal outcomes.
• Ensures implementation of therapeutic regimens and monitoring plans (care plans) by taking appropriate follow-up actions.
• Documents interventions and recommendations/clarifications in the electronic Health Information System as applicable.
• Detects, analyses, and reports adverse drug reactions.
• Designs execute or participate in a research project that may include data collection, statical analysis, andwrite research papers.
• Participates/assists in conducting performance/quality improvement projects.
• Actively participating in conducting medication use evaluation (MUE)
• Provides information and education resource for patients, students, colleagues, medical andnursingstaff, and other Healthcare professionals.
• Participates in self andothers' education, training and development if assigned

Pharm.

D./Bachelor’sDegree in Pharmacy is required
