RH Group is hiring for one Population Health Specialist. You will have the opportunity to indicate which location(s) you are interested in on the application.

We are seeking a detail-oriented and data-driven Population Health Specialist (LPN or RN) to join our Population Health team. This role is critical in improving health outcomes through preventive care strategies, patient outreach, and quality improvement initiatives. The ideal candidate will combine clinical knowledge with strong analytical and communication skills to support value‑based care programs, reduce disparities, and ensure accurate clinical documentation.

  • RESPONSIBILITIES Data Analysis & Reporting
    • Analyze population health data to identify care gaps, disparities, and trends in targeted communities.
    • Prepare reports and dashboards to inform clinical teams and support strategic decision‑making.
    Quality Improvement Collaboration
    • Work with Quality Improvement Specialists to develop and implement performance improvement initiatives.
    • Assist in monitoring and evaluating the effectiveness of programs to ensure compliance with healthcare regulations.
    Patient Outreach & Engagement
    • Conduct targeted outreach to patients for preventive screenings, chronic disease management, and care coordination.
    • Educate patients on services, promote health literacy, and encourage follow‑through on care plans.
    Chart and Diagnosis Audits
    • Perform routine chart audits to assess documentation accuracy, identify care gaps, and support coding compliance.
    • Conduct diagnosis audits to ensure accurate and complete capture of patient conditions for risk adjustment and quality reporting.
    • Collaborate with providers and staff to clarify documentation and support accurate diagnosis coding.
    Program Evaluation & Monitoring
    • Track outcomes using qualitative and quantitative metrics to measure the impact of health initiatives.
    • Adjust interventions based on findings to improve population health and operational efficiency.
    Value‑Based Care & Insurance Alignment
    • Attend payer and value‑based care meetings to ensure alignment of initiatives with program goals.
    • Review and maintain accurate insurance patient attribution lists
      , facilitating appropriate care coordination.
    • Upload documentation to payer portals to close care gaps and meet reporting requirements.
    Documentation & Communication
    • Maintain thorough and timely records of audits, patient interactions, and outreach activities.
    • Communicate findings and updates regularly to the QI Director and other relevant stakeholders.
    REQUIREMENTS Education
    • Bachelor’s degree in Nursing, Public Health, Health Administration, or related field preferred.
    • Current LPN or RN licensure required.
    Experience
    • 1–2 years in population health, quality improvement, care coordination, or clinical documentation review.
    • Prior experience conducting chart or diagnosis audits is highly desirable.
    Skills & Competencies
    • Strong attention to detail and knowledge of clinical documentation standards.
    • Proficiency in Excel and familiarity with EHR systems.
    • Excellent written and verbal communication skills.
    • Understanding of risk adjustment, social determinants of health, and health equity concepts.
    • Ability to work both independently and collaboratively in a fast‑paced environment.
    Additional Requirements
    • In‑person position with regional travel to clinical sites and community events.
    • Must have reliable transportation and be able to manage time across multiple locations.
