We are currently seeking an experienced HOSPICE INPATIENT ADMISSIONS NURSE.

JOB SUMMARY- Perform comprehensive hospice admission assessments in hospitals, hospice inpatient units, and contracted inpatient facilities. This role supports timely, compliant hospice admissions in inpatient settings while serving as a clinical liaison to referral sources, patients, families, and facility partners.
- Develop plans of care for patients admitted with the approval of the certifying physician.
- Coordinate patient care service as necessary amongst various disciplines and Clinical Managers.
- Document patient care data as required.
- Evaluate patients for hospice in inpatient hospitals and/or hospice inpatient facilities for which there are physician's orders, utilizing skills in physical assessment by systems review to completely assess/reassess the patient's condition and problems by identifying psychosocial and environmental factors related to patient care.
- Develop and update the plan of care as appropriate to the patient's diagnosis, HOPE findings, comprehensive assessment and condition, in coordination with the Clinical Manager.
- Make referrals to the appropriate disciplines and resources as ordered.
- Other job duties as assigned.
- Requires current RN licensure within state of branch location with at least two years of home health clinical nursing experience. Successfully completed nursing education from an approved school of nursing.
- Must be able to provide effective health care services to patients in their homes through patient evaluation (comprehensive assessment), care, planning and coordination. This includes thorough, accurate and complete documentation of the visit.
- Must accept travel and working conditions as assigned.
- Must be able to work independently and possess strong communication, organization and time management skills.
- Must be able to use a computer and have a working knowledge of a variety of computer applications such as Windows, Word, etc. CPR certification is required.
- Minimum of twelve (12) hours continuing education annually.
- Must have valid driver's license.
