Role Description

This is a full-time, on-site Social Worker role located in Roanoke, AL. The Social Worker will be responsible for assisting residents and their families with navigating healthcare and social services, conducting assessments, creating care plans, and coordinating with interdisciplinary teams. Additional responsibilities include providing emotional support, connecting residents to resources, and ensuring compliance with relevant policies and regulations to foster a supportive and caring environment.

• Proficiency in conducting assessments, developing care plans, and coordinating services for individuals
• Strong interpersonal, communication, and emotional support skills to assist residents and families
• Familiarity with healthcare policies, social service systems, and compliance requirements
• Demonstrated ability to work collaboratively with interdisciplinary teams
• Possession of a Social Work license and a relevant degree (e.g., Bachelor’s or Master’s in Social Work or a related field)
• Prior experience in healthcare, rehabilitation, or long-term care is a plus
• Ability to work independently and address challenges with professionalism
• Empathy, problem-solving skills, and cultural competency
