Job Summary

Home Health and Hospice RN Case Manager assists patients in the home or hospice setting by assessing, planning, implementing, and evaluating clinical care to achieve optimal outcomes.

Responsibilities
  • Assess, plan, implement, and evaluate patient care for patients of all ages in accordance with service area requirements.
  • Develop and update individualized care plans, including age‑specific assessments and documentation.
  • Collaborate with interdisciplinary team members, patients, and families to create, adjust, and evaluate care plans.
  • Administer and document treatments and medications using electronic medical records and automated computer systems.
  • Ensure patient care orders are current, accurate, and evaluated for effectiveness per provider order.
  • Provide patient/family (caregiver) education, incorporating it into the care plan.
  • Participate in performance improvement activities and advocate for patient rights and support.
  • Manage resources, establish priorities, and adapt the environment to meet patient needs.
  • Coordinate and conduct certified visits (OASIS, recertification) and implement BIPA/ABN forms or CMS‑directed forms.
  • Utilize Mifi device to access and share patient information during visits.
  • Perform supervisory duties as required (e.g., LPN or aide orientation).
  • Participate in case conferences and management meetings.
  • Assist in billing pre‑authorization and financial communication with patients.
Qualifications
  • Graduate of an accredited Registered Nurse program.
  • Valid RN license in the state of Virginia or an approved compact state.
  • Valid driver’s license, automobile insurance, and personal vehicle for home visits.
  • Experience:

    None required.
  • Keyboarding skills for electronic medical records and automated systems.
  • Strong critical thinking and prioritization skills, with knowledge of life‑span growth and development.
  • Ability to interpret CHCS, Carilion Clinic policies, and related laws and regulations.
  • Ability to work in a fast‑paced, patient‑care environment.
