At Good Samaritan Hospice, we consider it a sacred honor to walk with individuals and families through the end-of-life and grief journey. We are seeking a compassionate, strategic, and experienced leader to join our hospice team as the Director of Social Work .

In this role, you will lead and mentor a dedicated team of hospice social workers while providing clinical and administrative oversight of social work services across our service area. You will help ensure that patients and families receive exceptional psychosocial support, while also creating an environment where social workers can grow, thrive, and provide meaningful, patient-centered care.

• Provide leadership, clinical supervision, mentorship, and performance management for the hospice social work and counseling team.
• Oversee the quality and delivery of psychosocial services, ensuring assessments, interventions, and individualized plans of care are timely, appropriate, and patient-centered.
• Recruit, orient, develop, and evaluate social work staff while supporting professional growth and maintaining high standards of clinical practice.
• Coordinate staffing, caseload assignments, coverage, and continuity of care across the hospice service area.
• Ensure appropriate 24/7 on-call counselor coverage and provide ongoing supervision and support to on-call staff.
• Collaborate closely with the Interdisciplinary Group, Nursing, Spiritual Care, and other clinical leaders to address complex patient and family needs.
• Represent Social Work at the leadership level and participate in organizational committees, meetings, and interdisciplinary initiatives.
• Lead and support Quality Assessment and Performance Improvement (QAPI) initiatives, including chart audits, identifying trends, and developing performance improvement projects.
• Ensure departmental practices remain consistent with applicable regulatory requirements, Medicare Conditions of Participation, Virginia licensure requirements, agency policies, and professional ethics.
• Develop, review, and implement policies, procedures, and programs that support high-quality social work services.
• Build and maintain relationships with healthcare organizations, community partners, and local resources throughout Southwest Virginia.
• Facilitate ongoing education, team development, and department meetings to promote professional growth and strong clinical practice.
