Position Summary/Responsibilities

As a Mental Health Case Manager
, you will assist adults diagnosed with serious mental illness in accessing essential supports for living in the community and meeting basic needs.

A Day in the Life of a Case Manager:
Our Case Managers work with adults who have a primary diagnosis of a serious mental illness. The Case Manager provides case management services based on their client's individualized treatment plan. The highlight of a Case Manager's day is when a client is able to overcome a barrier.

Some of the services that may be performed during a typical day as a case manager are:

• Assessing Needs and Planning Services
• Coordination of Care with other Providers
• Linking to Community Supports
• Enhancing Community Integration
• Monitoring Service Delivery
• Supportive Counseling
• Educating The Client About Various Support Needs
• Advocating on Behalf of the Client
• Documenting services and treatment plan updates

Hiring Range

The anticipated base salary for this position is $43,022.07 to $45,890.21. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.

Requirements

• Education: High School diploma or GED required. An Associate's or Bachelor's degree in a human services field preferred.
• Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
• Safe Driving Record: This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.

Special Work Conditions

While performing the duties of this job, the employee will often sit, stand, walk, bend, stoop, and reach above their head. Use of hands for placing and pulling objects. The employee occasionally lifts and or carries up to 25 pounds and pushes or pulls up to 150 (wheelchair) pounds. Specific vision abilities required by this job include close vision (computer and research).

Specific hearing required by this job includes normal tones. Travel up to 5%. Travel is local. Travels to various sites in the community for the purpose of monitoring supports in a variety of settings and participating in meetings with peers and community agencies.
