Perform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents.

• Provide personal care—grooming, bathing, dressing, oral care—of residents daily and as needed.
• Change bed linens and maintain appropriate grooming supplies in resident rooms.
• Ambulate and transfer residents, using assistive devices and proper body mechanics.
• Record resident vital signs, weights, measurements, intake and output, food consumption, and other information as assigned.
• Routinely turn and position residents as necessary.
• Observe residents carefully and report changes in condition to Charge Nurse.
• Recognize residents’ ability to make choices and support these within the constraints of the care‑giving environment.
• Assist in collecting data for the MDS and plan of care; provide input to nurses for care plan development.
• Provide adequate hydration to residents at regular intervals according to specific needs.
• Prepare residents for meals, transport to dining rooms (unless a bed patient), and assist those who cannot feed themselves.
• Keep accurate records of food intake and output.
• Maintain housekeeping standards in assigned duty areas.
• Assist nursing staff with aseptic and sterile techniques to prevent infection.
• Ensure proper ventilation, temperature, light, and noise control.
• Measure TPR (temperature, pulse, rate), blood pressure, and weight as directed.
• Promptly answer call lights and assist with other resident needs.
• Utilize proper body mechanics when lifting or transporting residents.
• Assist in training and orientation of other staff as requested.
• Participate in process improvement initiatives and other special projects as assigned.

• Valid Nursing Assistant certification from state OR equivalent.
• Current CPR certification preferred.
• Effective verbal and written English communication skills.
• Intermediate to advanced skills with Microsoft Office (Word, Excel, PowerPoint, Outlook) and Internet navigation.
• Professionalism and ability to maintain confidentiality.
• Strong team orientation, customer‑service orientation, and ability to work well under pressure.
• Excellent attention to detail, organizational skills, and ability to prioritize multiple projects.
• Strong analytical and problem‑solving skills.
• Capability to work with minimal supervision and make independent decisions.
• Adaptability, flexibility, and personal integrity when working with staff and vendors.

• Heavy physical work, including lifting up to 50 pounds or more.
• Push, pull, move, or lift a minimum of 50 pounds to a height of at least 3 feet and a distance of at least 50 feet.
• Standing and/or walking for more than 4 hours per day.
• Bending and/or stooping for more than 1 hour at a time.
