RN Case Manager – Emergency Department – Geriatrics

Overview

Provides case management for assigned patient populations in the Emergency Department focused on geriatrics, ensuring optimal clinical and resource outcomes, cost‑effective care, and smooth transitions to other levels of care.

Responsibilities
  • Perform patient needs assessments upon admission and at regular intervals, facilitate referrals, and provide linkages to health, wellness, and post‑acute resources.
  • Promote interdisciplinary collaboration; participate in IDR morning and afternoon huddles; provide feedback and manage changes to the plan of care.
  • Use Inter Qual software and predictive analytic tools (e.g., JVION) to support accurate patient status, medical necessity, and discharge planning documentation.
  • Communicate with patients/families regarding financial implications of discharge plans and ensure understanding.
  • Coordinate referrals to DME, home health, hospice, ensure prior authorizations and co‑pay information are obtained.
  • Assist with regulatory letters, non‑coverage notices, and discharge planning documentation in compliance with CMS and state regulations.
  • Maintain close communication with Utilization Review Nurse and Social Work partner; review census daily; document status and barriers.
  • Advocate for patients and families throughout the episode of care; coordinate transitions to other acute facilities or jails/prisons.
  • Participate in departmental and system performance improvement initiatives; analyze data to assess impact of case management.
  • Provide medication assistance and investigation (RX Help, CMAP) for patients needing support.
Daily Tasks / Expectations
  • Attend IDR morning and afternoon huddles.
  • Coordinate referrals for DME, HH, Hospice.
  • Utilize predictive analytic software.
  • Complete face‑to‑face patient assessments.
  • Communicate with UR nurse and SW partner.
  • Reassess patients, document status of referrals, barriers.
  • Aid in delivery of regulatory letters (IM, HINN).
  • Integrate Inter Qual information during huddles.
  • Provide medication assistance; initiate investigations.
  • Communicate post‑acute care needs for inmates during transitions back to jail.
  • Assist in acute‑acute transitions of care.
  • Maintain awareness and anticipate unit‑based patient needs.
  • Provide hand‑off communication of unit needs to peers during weekday/weekend transitions.
Education & Experience

• Registered Nurse (RN) with current licensure in Virginia.

• Bachelor’s degree required; 5 years of RN experience in a hospital setting may satisfy the education requirement.

• Three years of recent experience in clinical health care with direct management of patient care, including planning, coordination, and delivery of services such as education, psychosocial support, discharge planning and utilization management.

Licensure & Certifications

• Current Virginia RN license.

• AHA BLS–HCP required within 6 months of hire.
