Geriatric Nurse Practitioner
Optum Home & Community Care, part of the Optum family, offers integrated care for senior patients.
Responsibilities- Conduct comprehensive assessments including past medical history review, medication reconciliation, vital signs, and physical exam.
- Provide evidence‑based screenings and point‑of‑care testing as appropriate.
- Support members during short‑term sub‑acute rehabilitation episodes.
- Ensure accurate documentation, including ICD‑10 coding.
- Communicate and collaborate with the interdisciplinary care team.
- Lead advanced illness and advanced care planning conversations.
- Offer counseling and education to patients and caregivers.
- For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
- For PAs: Current NCCPA certification and state licensure.
- Active, unrestricted state licensure (or ability to obtain by start date) without disciplinary actions.
- Valid, unrestricted driver’s license and reliable transportation.
- Active DEA license/prescriptive authority or ability to obtain post‑hire per state regulations.
- Ability to secure a collaborative practice agreement, if required.
- Physical ability to sit, stand, and kneel as needed.
- 1+ year clinical experience as an APC (long‑term care preferred).
- Experience in geriatric medicine, long‑term care, senior living or home care.
- Experience managing patients with complex behavioral, social, or functional needs.
- Understanding of geriatrics, chronic illness, and advanced illness/end‑of‑life discussions.
- Proficiency with electronic medical records and technology.
Sign‑on bonus: $3,500 for external candidates.
Hourly rate ranges from $52.64 to $78.85. Total cash compensation includes base pay and performance bonus, subject to local market, experience, and productivity.
