5 West is a 26‑bed, newly renovated unit at Carilion Clinic that specializes in meeting the complex needs of the medical and/or surgical patient. Our patient population ranges from pneumonia and general medical conditions to minor surgical procedures and complex medical/psychosocial needs. A special focus of our team includes the complex issue of IV drug related infections that require up to 6 weeks of medical care.

This special team consists of a Licensed Clinical Social Worker, Peer Recovery Specialists, Addiction Physicians, Psychiatric Technicians, Clinical Nurse Specialists, and many others who are passionate about helping in the recovery of patients who have often lost hope for a future life. Our patients have lovingly named our unit “The Hope Unit”.

• Practice practical nursing within the parameters set forth in Carilion Clinic’s plans for the provision of patient care and function under the direction of a registered nurse or physician.
• Partner with the RN in collecting data, planning, implementing, and evaluating patient/resident care.
• Participate in the nursing process for patients of all ages served.
• Communicate frequently with team members, ancillary departments and physicians.
• Contribute to the development of the patient/resident database and interdisciplinary plan of care.
• Identify patient/resident/family/caregiver educational needs, incorporate education into the plan of care, and document.
• Apply knowledge of growth and development to appropriately care for patients/residents.
• Implement prescribed therapeutic treatment plans and procedures.
• Administer prescribed medication to patients/residents.
• Advocate for patients as protector of patient’s rights and provider of psychological/social/cultural support.
• Contribute to Carilion’s performance improvement activities and support service area/facility/organization initiatives.
• Contribute to the orientation and career development of others and may assign tasks as needed.

• Education:
  
  Graduate of an accredited school of practical nursing.
• Licensure:
  Licensed to practice practical nursing in the state of Virginia.
• De‑escalation training within four months of hire and maintained thereafter (required in Psychiatry and Behavioral Health).
• Demonstrates knowledge of the principles of growth and development and ability to collect data reflective of the patient’s status, interpret the appropriate information to identify each patient’s requirements relative to his or her age and specific needs, and provide the care needed as described in departmental policies and procedures.
