Position Summary
The Nurse (LPN) (RN) provides direct nursing care and clinical oversight in a DBHDS-licensed Residential Crisis Stabilization Unit (RCSU) for individuals experiencing acute psychiatric or behavioral health crises. The nurse is responsible for conducting health assessments, administering and monitoring medications, supporting crisis stabilization interventions, monitoring physical health conditions, and collaborating with the interdisciplinary team to promote safety, stabilization, and recovery. The nurse ensures all services are delivered in accordance with Virginia DBHDS
, Virginia Medicaid (DMAS), and agency policies.
- Complete nursing assessments during admission and provide ongoing evaluation of physical and behavioral health status under the direction of the NP.
- Administer prescribed medications and treatments, monitor for side effects, and document all medication administration in the MAR.
- Assist prescribers (physicians, NPs, or PAs) with psychiatric and physical healthcare needs, including medication reconciliation and lab coordination.
- Maintain accurate and timely nursing documentation in the client’s medical record, including vitals, medications, interventions, and clinical observations.
- Provide education to clients regarding medications, physical health, wellness, and self-care practices.
- Collaborate with clinical staff (LMHPs, QMHPs, peers) and medical providers to ensure integrated care planning and discharge coordination.
- Respond to medical or psychiatric emergencies following agency protocols, including use of first aid, CPR, or other emergency interventions.
- Monitor for safety risks, including suicide precautions, medical deterioration, and withdrawal symptoms.
- Participate in interdisciplinary team meetings and contribute to Treatment Plan/ individualized service plans (ISPs).
- Maintain inventory of medications, first aid supplies, and controlled substances per agency and regulatory standards.
- Ensure adherence to infection control, medication management, and OSHA requirements in a residential setting.
- Submit all clinical documentation (progress notes, assessments, treatment plans, quarterly reports) within 24 business hours in accordance with Virginia Medicaid guidelines.
- Maintain compliance with DBHDS, Office of Human Rights, and OSHA standards.
- Report any incidents of low engagement, behavioral concerns, or suspected abuse/neglect to appropriate personnel.
- Active and unrestricted Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the Commonwealth of Virginia.
- Minimum of one (1) year of nursing experience in behavioral health, emergency services, community-based, addiction recovery & treatment, or residential treatment setting preferred.
- Current CPR/First Aid certification (BLS preferred), Behavior Modification
- Medication administration training per DBHDS/agency standards.
- Valid Virginia driver’s license with current DMV driving record.
