Job Overview
Clinical Coordinator opportunity for RN managing a CRRT program with Inpatient Dialysis. Full time, primarily day shift. The Clinical Coordinator uses advanced nursing processes in decision‑making, patient assessment, data analysis, development, and implementation of methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. Provides direct and indirect patient care as needed;
oversees the delivery of patient care; and provides day‑to‑day management of programs to coordinate patient care as designated by the department.
- Coordinates the delivery of safe, high‑quality patient care and identifies areas for study.
- Participates in and/or supports research initiatives.
- Utilizes advanced nursing processes in clinical decision‑making, patient assessment, and care coordination to improve quality, efficiency, and patient outcomes for the assigned population.
- Coordinates interdisciplinary care plans, ensuring alignment with patient needs, clinical guidelines, and organizational goals.
- Performs patient assessments, facilitates referrals, and ensures appropriate placement and transitions across the continuum of care.
- Collaborates with physicians, nursing staff, social work, and ancillary services to ensure coordinated and effective patient care.
- Identifies and addresses barriers to care and implements strategies to improve patient outcomes, including length of stay and resource utilization.
- Develops, analyzes, and evaluates clinical and operational data to identify trends and support quality and performance improvement initiatives.
- Ensures compliance with regulatory and accreditation requirements, including The Joint Commission (TJC) standards.
- Provides education to patients, families, and staff to support understanding of care plans, treatment options, and post‑discharge needs.
- Serves as a resource, mentor, and educator to staff and team members.
- Facilitates communication among patients, families, and care teams to ensure continuity and quality of care.
- Participates in interdisciplinary team meetings and contributes to care planning and progression discussions.
- Supports departmental and system‑wide performance improvement and research initiatives.
- Maintains accurate and timely documentation in accordance with organizational and regulatory standards.
- Advocates for patients and families throughout the care experience.
Education: Graduate of an accredited School of Nursing required. Bachelor’s degree preferred.
Experience: Minimum of 3 years of RN experience required. Experience in a relevant specialty or department preferred, as defined by the department.
Licensure, Certification, and/or Registration: Current licensure as a Registered Nurse (RN) in the State of Virginia required. AHA BLS‑HCP certification required or must be obtained within 90 days of hire. Additional certifications (e.g., ACLS, CCRN, ONC, Pain Management, or other specialty certifications) may be required based on department.

Minimum Qualifications:
Valid driver’s license required for roles involving travel, such as Home Health. Strong interpersonal, communication, and teamwork skills. Demonstrated critical thinking, conflict resolution, organizational, and effective judgment abilities. Ability to complete orientation and competency validation requirements. Must be able to meet essential job functions. Knowledge of Medicare, Medicaid, and other third-party payor regulations may be required based on department. De‑escalation training must be successfully completed within one year of hire and maintained thereafter.
Compensation Range: $69,721.60 – $. Employee pay is based on a variety of factors, including qualifications, credentials, education, and prior work experience. The compensation range does not include benefits or pension plan.
