Home Health RN Case Manager:
Job Summary

The Home Health RN Case Manager administers skilled nursing care to patients requiring intermittent professional services. This role focuses on coordinating, implementing, and evaluating care in the home setting in accordance with physician orders and individualized plans of care. The RN also provides patient and family education to promote independence, safety, and optimal health outcomes.

What you will do as a Home Health RN Case Manager:

Conduct skilled nursing visits in patients’ homes
Complete admissions, re-certifications, recertifications, and discharge assessments
Develop, implement, and revise Plans of Care based on patient needs
Complete OASIS documentation accurately and within required time frames
Coordinate care with physicians, therapists, and interdisciplinary team members
Provide patient and caregiver education on disease process, medications, and self-care
Monitor patient progress and adjust care plans as needed
Participate in case conferences, on-call rotation, and administrative meetings

What we are looking for in a RN Case Manager:

Graduate of an accredited nursing program
Minimum of 2 years nursing experience (home health or hospice experience, acute care, or geriatrics preferred; OASIS experience strongly preferred (home health)
Active Virginia RN license (or compact license)
Current BLS/CPR certification or obtained within 30 days of hire
Strong clinical assessment, communication, and decision-making skills
Comfort with EMR systems and mobile documentation tools
Valid driver’s license, reliable transportation, and proof of insurance

Schedule & Benefits:

Schedule: Monday–Friday, approximately 8:30 AM – 5:00 PM (with participation in on-call rotation as required)

Location: Roanoke and surrounding service areas
