How You’ll Help Transform Healthcare:
The LPN Patient Flow Enhancement supports patient flow and pre-admission processes by coordinating patient information, preparing documentation, and assisting with care planning activities. Collaborates with clinical and administrative teams to ensure efficient patient transitions and high-quality patient care. The LPN Patient Flow Enhancement Supports patient flow and pre-admission processes by preparing patient charts, verifying documentation, and coordinating required information prior to admission.
Communicates with physician offices, laboratories, and patients to obtain necessary clinical information and test results. Educates patients via telephone regarding procedures, pre-admission instructions, and unit expectations. Assists with data collection to support the nursing process and care planning activities. Maintains and organizes patient records, files, and documentation in accordance with organizational standards. Provides administrative support including scheduling, meeting coordination, report preparation, and data tracking.
Monitors patient schedules to identify overbooking or duplication and communicates concerns to appropriate staff. Supports performance improvement initiatives and contributes to quality care outcomes. Communicates effectively with interdisciplinary teams to ensure smooth patient transitions. Performs additional duties related to departmental operations as assigned. Demonstrates professionalism, respect, and commitment to service excellence in all interactions.
Education:
Graduate of an accredited school of practical nursing required.
Experience:
Minimum of 2 years of LPN experience required. Previous administrative or clerical experience preferred.
Licensure, Certification, and/or Registration:
Current licensure as a Licensed Practical Nurse (LPN) in the Commonwealth of Virginia required or eligibility to obtain.

Minimum Qualifications:
Strong organizational, communication, and multitasking skills. Ability to work independently and manage competing priorities. Proficiency with Microsoft Office and electronic medical record systems required.

Compensation Range: $23.23 - $32.52 Employee pay is based on a variety of factors, including qualifications, credentials, education, and prior work experience. The compensation range does not include benefits or pension plan.
