Home Health - RN Case Manager

Overview

Sanford Health – a leading rural health system – is seeking a field‑based home health nurse to provide direct patient care in patients’ homes. The role includes managing a caseload, performing skilled nursing visits, conducting assessments and education, coordinating care, and documenting outcomes.

• Manage own patient caseload while completing skilled nursing visits, assessments, patient education, care coordination, and documentation in the home setting.
• Collaborate closely with LPNs and an interdisciplinary team to ensure high‑quality patient outcomes and coordinated care throughout the episode.
• Integrate health care from pre‑admission to post‑discharge by coordinating and sequencing patient care, enhancing flow, and promoting early intervention.
• Utilize structured care methodologies, protocols, and integrated clinical pathways across all age groups as appropriate.
• Communicate effectively with patients, families, and clinical team members to facilitate care planning and problem‑solving.
• Document care clearly, monitor patient responses, and use data to evaluate trends and outcomes.
• Exercise independent nursing judgment, assertiveness, and critical thinking in all patient interactions.
• Control variances, manage duplication, and maintain knowledge of reimbursement for services.
• Work primarily day hours, scheduled weekends, and occasional evenings; may be exposed to communicable disease, hazardous materials, and other stressors.

• Bachelor's degree in Nursing from an accredited program (CCNE/ACEN/NLN CNEA) – preferred.
• Minimum of two years clinical experience – preferred.
• Current RN license in the state of practice; multistate licensure acceptable through the Nurse Licensure Compact.
• Ability to obtain and maintain required department‑specific competencies and certifications.
• Valid driver’s license may be required depending on location.
• Self‑motivated, works with little direction, and demonstrates strong flexibility and organizational skills.
• Excellent communication skills and ability to teach and adapt education to age groups.
• Knowledge of reimbursement and billing for home health services.

• Salary $34.00–$51.00 per hour with potential sign‑on bonus up to $10,000.
