About the Facility
Good Samaritan Society – Specialty Care Community is a 96‑bed not‑for‑profit facility that has provided housing and services to seniors for over 100 years. It offers rehabilitation therapy in both young adult and geriatric communities following hospitalization or serious illness. The facility has three floors divided into two self‑contained wings, each housing 16 residents in private rooms with wheelchair/lift‑accessible showers and a shared dining area and nurses’ station.

The Clinical Care Leader (CCL) utilizes the nursing process—assessment, diagnosis, outcomes/planning, implementation, and evaluation—to provide individualized nursing care in the home setting. The role collaborates with inter‑professional colleagues, documents resident history, medications, and care provided, and employs technology to enhance resident quality of care. In addition to direct patient care, the CCL delegates and supervises unlicensed personnel, assumes overall nursing accountability for resident assignments, and serves in leadership roles on departmental and organizational task forces and committees.
Key Responsibilities- Apply the nursing process to design and implement individualized care plans for residents in a home setting.
- Document resident history, symptoms, medications, and care activities in compliance with applicable standards and regulations.
- Collaborate with inter‑professional team members, effectively delegate to and supervise unlicensed staff, and ensure optimal coordination of care.
- Lead on department and organizational task forces, committees, and initiatives.
- Maintain professional communication to resolve conflicts and negotiate with peers, residents, families, and colleagues.
- Analyze data and demonstrate clinical reasoning and critical thinking to respond to evolving resident needs.
- Act within the scope of nursing practice as defined by the Nurse Practice Act and administrative rules of the state of licensure.
- Bachelor’s degree in Nursing strongly preferred.
- Minimum of two years of clinical experience preferred.
- Current, unencumbered RN license in the state where practice occurs; multistate licensure acceptable in NLC states.
- Basic Life Support (BLS) certification required.
- Valid driver’s license may be required depending on location.
- Experience with Resident Assessment Instrument (RAI) process is preferred.
- Strong communication, collaboration, and conflict‑resolution skills.
- Demonstrated critical thinking and clinical reasoning abilities.
