Clinical Manager – RN, Home Health

• Manage nursing staff and nursing activities for an assigned home health and/or hospice department
• Oversee patient/resident care, record-keeping, budgeting, and personnel issues
• Assign patients/residents to nursing staff based on condition and employee skill level
• Schedule nursing staff to specific shifts using utilization trends
• Measure employee performance
• Train new staff and develop existing staff
• Manage workflow volumes, including monitoring and processing work queues
• Apply continuous improvement processes and evaluate quality impact
• Monitor patient/resident care areas and facilities for cleanliness and readiness
• Provide nursing care on an as-needed basis
• Direct nurse and other staff activities for home health and/or hospice services
• Ensure effectiveness and efficiency of home health and/or hospice services
• Delegate responsibilities to clinical and professional staff
• Provide direction on nursing practice and home care/hospice regulations
• Provide input for planning and budgeting
• Coordinate interdisciplinary communication and collaboration
• Liaise internally and with outside agencies
• Assist with and manage special projects assigned by leadership, championing enterprise initiatives

• Bachelor's degree in nursing required
• Graduate from a nationally accredited nursing program preferred, including CCNE, ACEN, and NLN CNEA
• Thorough knowledge of the nursing practice act and standards of nursing care
• Working knowledge of administrative and management techniques
• Two years of nursing clinical experience preferred
• Two years of experience in management duties preferred
• Currently holds an unencumbered Registered Nurse (RN) license with the State Board of Nursing where nursing practice occurs, or possess multistate licensure in a Nurse Licensure Compact state
• Valid driver's license may be required depending on location
• Basic Life Support (BLS) may be required for frequent patient contact and patient care
• Certified Hospice and Palliative Nurse (CHPN) desirable
• Certificate for Outcome and Assessment Information Set (OASIS) Specialist-Clinical (COS-C) preferred

Demonstrates strong leadership in managing nursing staff and overseeing patient care within home health and hospice settings, while ensuring compliance with nursing regulations and quality standards. Proficient in staff training, performance evaluation, and interdisciplinary collaboration to enhance service effectiveness.
