The Nurse Practitioner will lead the onsite multidisciplinary care team while providing quality patient care and driving positive patient outcomes in accordance with Vigilant Health's clinical model. This role also participates in the supervision, training, and development of clinical staff to support the effective management of plan members and the implementation of customized well-being programs developed for each client organization.

Essential Duties and Responsibilities
  • Provide comprehensive patient care by interviewing, examining, diagnosing, and treating patients in accordance with established Vigilant Health protocols.
  • Educate patients and their families on wellness, prevention, chronic disease management, and early intervention through individualized counseling and educational resources.
  • Determine the appropriate level of urgency for follow-up care, referrals, and specialty consultations.
  • Prescribe appropriate medical treatments and medications in accordance with Vigilant Health clinical guidelines and under the direction of the Chief Medical Officer.
  • Demonstrate effective population health management and engagement of assigned plan members.
  • Assure appropriate provider coverage is maintained during clinic operating hours.
  • Document all patient encounters and services accurately and timely within the electronic medical record.
  • Promote and integrate client-specific well-being and population health initiatives into patient care whenever appropriate.
  • Collaborate with clinic physicians and other members of the care team regarding patient care, assessments, treatment plans, and educational opportunities.
  • Assist in the resolution of patient concerns, complaints, and inquiries in a professional and service-oriented manner.
  • Maintain strict confidentiality of patient information and comply with all applicable federal, state, and organizational privacy requirements.
  • Function effectively with a high level of autonomy while maintaining strong collaboration with clinical leadership and team members.
  • Support clinic operations through staff supervision, mentoring, coaching, and other leadership responsibilities as needed.
Qualifications
  • Minimum of three (3) years of Nurse Practitioner experience required.
  • Master's Degree in Nursing required.
  • Current Nurse Practitioner certification and licensure.
  • Current DEA registration.
  • Demonstrated leadership abilities and experience working within a multidisciplinary healthcare team.
  • Strong commitment to patient-centered care, prevention, wellness, and chronic disease management.
  • Excellent communication, interpersonal, and relationship-building skills.
  • Ability to work independently while contributing to a collaborative team environment.
