Physical Therapist
The Physical Therapist is responsible for evaluating, planning, directing, and administering Physical Therapy services as prescribed by a licensed Provider. Services are provided within an inpatient, outpatient, and/or home health setting. Treatments aim to restore function, improve mobility, relieve pain, prevent or limit physical disabilities, and increase independence to enhance overall functioning. The Physical Therapist treats patients using various modalities and skilled techniques.
Patients are instructed on the use of adaptive and assistive equipment. Supervision and direction of Physical Therapy Assistants occur as needed. Maintaining open communication with the Rehab team is essential. Consultation with other departments may be appropriate to achieve positive clinical outcomes. An appropriate assessment on all patients is performed per physician orders. Results are interpreted within the scope of licensure.
Treatment programs are designed for patients with varying degrees and types of disabilities. These programs also address multiple medical conditions in accordance with accepted practice patterns and physician orders. Realistic short- and long-term treatment goals are formulated for each patient. Patient and family involvement in the plan of care development is included. Treatment effects are observed and evaluated, with recommendations made to the physician as necessary.
Accuracy in patient charges and documentation is ensured. Documentation must be timely and compliant with regulatory standards. Knowledge of computerized documentation methods is necessary. Documentation includes evaluations, progress notes, monthly notes, and discharge summaries. All duties are performed in an environment that prioritizes patient safety. Compliance with established safety procedures is required to ensure a safe environment for everyone involved.
Participation in performance improvement activities is necessary. Additional duties may be assigned as required.
