QA Registered Nurse (RN) – Home Health Quality Assurance
Salary: $80,000 - $115,000 per year (based on experience)

This is an excellent opportunity for a Home Health nurse who enjoys ensuring clinical excellence, regulatory compliance, and quality patient care behind the scenes.
This position is fully in-office and works closely with clinical leadership, field staff, and operations teams to ensure documentation accuracy, Medicare compliance, and successful patient outcomes.

Responsibilities- Review and audit Home Health clinical documentation for accuracy and compliance
- Perform QA review of:
- Start of Care (SOC)
- Resumption of Care (ROC)
- Recertifications
- Discharges
- Evaluations and reassessments
- Review and approve physician orders and care plans
- Review and process 45-day summaries
- Ensure compliance with Medicare, State, and Accreditation standards
- Identify documentation deficiencies and communicate corrections to clinicians
- Work within Home Care Home Base (HCHB)
- Assist with quality improvement initiatives
- Support branch offices and clinical teams with documentation compliance
- Active Registered Nurse (RN) License
- Minimum 2 years of Home Health experience
- Strong understanding of Medicare Conditions of Participation
- Experience reviewing OASIS documentation
- Excellent attention to detail
- Strong communication and organizational skills
- Previous QA, Clinical Review, Clinical Manager, or Case Management experience
- Home Care Home Base (HCHB) experience
- OASIS certification or advanced OASIS knowledge
