Case Manager​/Mental Health Practitioner - Harmony, MN

Base pay range: $24.37/hour - $30.00/hour.

We are looking for an individual to provide Case Management and Adult Rehabilitation Mental Health Services (ARMHS) services to people with severe mental illness. The position will provide direct care, both individually and in group settings, in the community, client homes, or in the office.

This position is located in Harmony, MN.

• Maintain a caseload of clients.
• Facilitate services to clients individually and in a group setting.
• Coach, teach, educate, and model independent living skills to clients.
• Accurately complete all required documentation and notes for each client in a timely manner.
• Coordinate and make referrals for client services that will help the individual meet their goals.
• Work collaboratively with all members of the treatment plan as often as needed.
• Follow-up on referrals and monitor the client's progress.
• Provide crisis assessment and intervention services to clients as needed.

• Bachelor's degree in Social Work, Psychology, Sociology, or a related Human Services field.
• Meet the State definition of Mental Health Practitioner.
• Two years of experience working in mental health strongly preferred.
• Knowledge of resources in Fillmore County.

Pay: $24.37 - $30 per hour. Compensation is based on years of experience.
