WHAT DO WE DO?CHSI is a private, non-profit organization established in 1973 to serve the health needs of migrant & seasonal farm workers and their family members. Today, CHSI has expanded services and provides mental & physical healthcare to all individuals in our communities. Our services are provided on a more affordable sliding fee scale to those who are under-insured or un-insured. We also accept ND and MN Medicaid, Medicare, and most private insurance plans.
We currently operate four nurse-managed clinics, two mobile medical units, and two victim advocacy program centers in Minnesota & North Dakota.
We are on the search for a full-time MN Sure Navigator (Outreach and Enrollment Specialist) to join our Rochester, MN team. In a MN Sure Navigator (Outreach and Enrollment Specialist) role at CHSI, you would work to provide outreach opportunities for community residents who lack access to healthcare and health insurance. These services may include:
- Provide enrollment assistance (including but not limited to completing insurance applications, gathering required documentation and troubleshooting the enrollment process) for uninsured children and adults to access subsidized, low-cost and free health insurance programs through the health insurance marketplace, Medicaid, and Children's Health Insurance Program (CHIP).
- Provide structured patient education, on health coverage, engage in follow-up conversations and offer renewal assistance for enrolled individuals.
- Distribute outreach materials to patients, clients, community members, partner organizations and business to build coverage option awareness.
- Develop, discover and attend community events in order to promote coverage options and the mission and services of the organization.
- Collaborate with various local organizations to build awareness of coverage options, spur enrollment and build referral linkages.
- Develop and assist in the presentation at staff training sessions; arrange and coordinates community group meetings and support group sessions.
- Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
- Accurately provide required reporting to track goal achievement and client satisfaction.
- Assist in the development and implementation of organizational outreach and enrollment initiatives.
- Perform other duties essential to the position and program
In addition to being a wonderful team member and health insurance specialist to clients, we are seeking an individual with the below qualifications:
PHYSICAL ABILITIES- In an eight hour workday, the employee must be able to: bend and crouch occasionally,
sit continuously and stand or walk frequently. - Employee must be able to lift up to 50 pounds occasionally.
- Employee must be able to use hands/arms, feet/legs for repetitive fine to large motor skill movements
- High school graduate or G.E.D. equivalent preferred. An equivalent combination of education and experience will be considered.
- Must have own transportation, current driver's license, and auto insurance.
- Ability to communicate and write legibly in Spanish and English and interpret documents, write routine reports and correspondence.
- Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, internet-based applications and the Microsoft operating system.
- Experience working with social service or other community agencies advantageous.
- Familiar with locations and services provided by local agencies desirable.
- Ability to work flexible hours, including evening hours.
- Knowledgeable about medical terminology and human anatomy very helpful, but not required.
