Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) +
Mileage Reimbursement

As a Physician Assistant focused on wound care and senior living medicine , you'll work with patients who often face complex medical challenges, chronic conditions, and barriers to recovery. You'll have the opportunity to identify concerns early, intervene proactively, and help patients avoid complications that can significantly impact their quality of life.

• Provide on-site rounding care throughout assisted living, memory care, and long-term care communities.
• Manage pressure injuries, diabetic ulcers, post-surgical wounds, and skin integrity concerns.
• Support chronic disease management and preventative care initiatives.
• Collaborate with physicians, nurses, therapists, caregivers, and interdisciplinary teams.
• Communicate effectively with families, caregivers, and facility staff.
• Complete documentation, medication management, diagnostics, and lab coordination.

• Active Minnesota Physician Assistant (PA-C) license in good standing.
• Experience in wound care, geriatrics, primary care, internal medicine, family medicine, or post-acute care preferred.
• Strong clinical judgment and communication skills.
• Valid driver's license and reliable transportation.
• Commitment to high-quality, patient-centered senior care.
