Humana Center Well Home Health is seeking a compassionate Home Health Aide to support patients in their homes under RN supervision. You will assist with personal care, mobility, nutrition and daily activities in a patient-centered plan of care.
The role requires an active CNA/HHA license where applicable, a valid driver's license and reliable transportation, and at least one year of HHA/CNA experience in clinical settings. This is a patient-facing, field-based position in Minnesota.
