Registered Nurse - Field Assessor

We are hiring a PRN Field Assessor (Registered Nurse Field Assessor) in Rochester, MN. The role involves completing Long Term Care insurance assessments, obtaining claimant demographics, physician information, a current medication list, and evaluating functional independence, ADL/IADL tasks and cognitive screening. Candidates must be technologically savvy and able to use a point‑of‑care web application for real‑time documentation.

• Current and unrestricted RN licensure in the state of practice
• Current driver’s license, vehicle insurance and access to a dependable vehicle or public transportation
• Current CPR certification
• Demonstrated ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

• 1+ years of RN experience
• Demonstrated ability to work independently
• Proven good communication, writing, and organizational skills

$64,100 - $141,500 annual total cash target pay. $36.98 - $81.63 per visit point. $30.82 - $68.03 hourly rate.

Annual total cash compensation assumes full‑time employment (40 hours per week) at full productivity and generally follows the range above. Compensation includes earnings from per visit point pay and hourly pay. Potential for increase over time based on productivity and performance. We comply with all minimum wage laws as applicable.
