RN Clinical Nurse Supervisor POSITION SUMMARY

The RN Clinical Nurse Supervisor (CNS) is responsible for leading and overseeing the delivery of clinical care within the Assisted Living setting. This role ensures high-quality, person-centered services in compliance with the Minnesota Nurse Practice Act, Assisted Living Licensure regulations (Minn. Stat. § 144G), and applicable organizational policies. The CNS manages care planning, supervises nursing and unlicensed personnel, and plays a key role in regulatory compliance, interdisciplinary collaboration, and staff development.

ESSENTIAL

RESPONSIBILITIES AND DUTIES CLINICAL OVERSIGHT AND RESIDENT CARE
  • Conduct comprehensive nursing assessments upon admission and at required intervals, revising care and service plans accordingly.
  • Review, evaluate, and update individualized Service Agreements based on changes in resident condition or preferences.
  • Ensure clinical services align with provider orders, facility policies, and nursing best practices.
  • Monitor and respond to resident condition changes; coordinate appropriate medical or supportive interventions.
  • Collaborate with physicians, families, case managers, and external providers to support care coordination.
  • Oversee infection control and wound care protocols; participate in quality assurance and clinical risk mitigation strategies.
  • Provide on-call support for urgent clinical issues and guide staff through emergent care protocols.
SUPERVISION AND STAFF DEVELOPMENT
  • Supervise and mentor LPNs, CNAs, and Unlicensed Personnel (ULPs); conduct performance evaluations and provide coaching as needed.
  • Oversee medication administration practices and ensure documentation meets clinical and regulatory standards.
  • Participate in employee recruitment, onboarding, clinical orientation, and ongoing training, including health and safety education.
  • Cultivate a culture of psychological safety, wellness, and accountability among staff; address performance issues proactively and constructively.
REGULATORY COMPLIANCE AND DOCUMENTATION
  • Ensure facility practices adhere to Minn. Stat. § 144G, § 245D (as applicable), DHS, MDH, and Board of Nursing guidelines.
  • Prepare for and lead responses to regulatory audits, inspections, and surveys.
  • Maintain accurate and timely documentation, including incident reporting, medication audits, resident records, and staff training logs.
  • Monitor charting practices, ensure documentation integrity, and implement corrective measures when needed.
TEAM COLLABORATION AND LEADERSHIP
  • Serve as a clinical liaison between nursing services and other departments, leadership, residents, and families.
  • Promote resident-centered care through leadership presence, open communication, and care conference participation.
  • Attend and contribute to interdisciplinary meetings, strategic planning sessions, and community engagement activities.
  • Support the facility’s mission, vision, and values by fostering a professional, collaborative, and compassionate environment.
QUALIFICATIONS Required
  • Current, unencumbered RN license in MN
  • Minimum 2 years of nursing experience, preferably in assisted living, skilled nursing, or community health.
  • Demonstrated supervisory experience with nursing or direct care staff.
  • Knowledge of Minnesota Nurse Practice Act, 144G/245D regulations, and relevant healthcare standards.
  • Strong interpersonal, leadership, and clinical judgment skills.
Preferred
  • Education or coursework in healthcare administration or leadership
  • Bachelor of Science in Nursing (BSN)
  • Experience with Electronic Health Records and digital care documentation platforms.
